The Benin University Hospital (UBTH) is now selling admission forms to its three (3) year basic midwifery program for the 2021/2022 academic session. See the requirements and how to apply for UBTH School of Midwives Form below.

Applications from the general public are invited for admission to the School of Midwives of the Benin University Hospital (UBTH), Bénin Ville for the 2021/2022 academic session.

UBTH Midwifery School Admission Qualifications

You must have the following qualifications to apply for the three (3) year UBTH Midwifery Program.

  • Must / must not be under 18 or over 34
  • Possess five (5) credits in no more than two sessions in WAEC / NECO, NABTEB, SSCE / GCE / OL ‘which must include, English language, mathematics, biology, physics and chemistry.

How to Apply for UBTH Midwifery School Admission Form

Application forms can be obtained from the office of the principal / head of department (midwifery school) upon presentation of proof of payment of the non-refundable fee of Ten thousand five hundred naira (10,500 N) payable only to the UBTH account with the TSA under the type of service: Midwifery training school using the REMITA platform at any bank in Nigeria.

  • Payment of two hundred (200) naira to the account of the State Committee of the Council of Nurses and Midwives of Nigeria – First Bank 2004854377

NOTE THE FOLLOWING:

  1. The sale of forms begins on June 17, 2021 and ends on September 17, 2021.
  2. All forms should be purchased from the Office of the Director / Head of Midwifery School, UBTH.
  3. Please bring the original and photocopies of your credentials.
  4. Candidates would be responsible for their accommodation and food during the exam period.
  5. Please be aware that UBTH does not sell School of Midwifery application forms online.

SUBMISSION OF THE APPLICATION FORM: Completed application forms must be submitted to the institution prior to the date of the entrance examination.

Entrance exam date: Saturday, September 18, 2021
Location: School of Midwives, UBTH, Benin City.

Please bring the original and photocopies of your credentials,

  1. Go to http://www.sis.zamsut.edu.ng/
  2. Enter your username and password in the required columns (your JAMB registration is your username and password)
  3. Click on the “Applicant” tab then on “Proof of admission”
  4. If accepted, click on the link ‘Click here to verify your payment to generate Remita payment invoice’.
  5. Bring the bill to the bank for payment
  6. Return to this page after payment to verify your payment by clicking on the link Click here to verify your payment acceptance ‘
  7. If your payment is verified successfully, you will be able to print your invoice

The management of Zamfara State University, Talata-Mafara (ZAMSUT) has published the registration procedure for students admitted during the 2020/2021 academic session.

ZAMSUT registration procedure for newly admitted students 2020/2021

  1. Candidates admitted to Zamfara State University, Talata Mafara are: first and foremost. required to connect to http://www.sis.zamsut.edu.ng/ using their Jamb registration number as “username” and “password
  2. when logging in, candidates must click on “My account” to change their password if necessary, otherwise go to the next step:
